Your First Visit

Patient Forms

Upon entering our office our receptionist will greet you, welcome you and thank you for choosing  Kingston Chiropractic & Rehab chiropractic provider.  If you have not already done so via this website we will request you fill out some basic paperwork. The paperwork provides us with general information about yourself and your condition.


Consultation

Next you will have a consultation with Dr. Wertman to discuss your health-related problems, concerns, as well as potential treatment options. Your first visit is designed for Dr. Wertman to learn more about you, your condition,  your expectations and to determine whether chiropractic care is the reatment of choice for your condition.


Examination

After your consultation, Dr.Wertman will perform a complete examination testing your ability to turn and bend as well as other standard neurologic, orthopedic, postural, and physical examination procedures. Nothing will be done in our office without your consent


Diagnostic Studies

Your specific condition may require us to send you for diagnostic imaging prior to beginning any treatment. If we find diagnostic imaging is necessary prior to starting any treatment, our office will refer you to one of the many imaging facilities throughout the Wyoming Valley. Our receptionist will make all the necessary phone calls and appointments to meet your needs.


Report of Findings

Once all the information and examinations have been performed, Dr. Wertman will give you a report of findings to answer the  most popular questions:

  1. Can you help me?
  2. What do you recommend that I do to get better?

After discussing with you your history and your goals, examining your spine and reviewing any diagnostic imaging or tests, Dr.Wertman will discuss recommendations with you. If you have a condition that requires care with other providers, the doctor will tell you. Your condition may also require the care of two differnt types of providers. If  Dr. Werttman believes we can help you, we will recommend treatment.


Treatment

Patients will generally be provided with their first treatment at the conclusion of the appointment. This may include spinal adjustments, rehabilitative exercises, and/or soft tissue therapies.  Treatment is interactive with you, so you can express any concerns you have, and what treatment you maybe most comfortable with.


Home Care Program

Prior to leaving, patients may be instructed on a home care program to be conducted at home. If you are in pain when you first come into our office this may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.   Every body is unique and different and therefore everone requires a unique and customized home care plan.  If you have any questions regarding any part of your day at the office they can and will be answered at this time.
